find a 'sms gateway' and get a subscription or buy sms-credits from them. Then you can use their interface to send sms's through.

It's really simple in fact - I send an sms by constructing 1 url which i send to the sms gateway. And that's that. The url contains all the info about who the sender is, who the recipient is and the message to send.
Then they have a reports page where i can request information about any sms that has been sent (whether it has arrived or not and such). This I store in our own db for quick lookups at any time.

That's it basically.

oh, you have to use a sms gateway, because getting your own direct SMS interface with a telco is extremely expensive and requires a special dedicated line. So you want to use a gateway instead, unless you're like Big Brother or so, then it makes sense to have your own line.

we do the same at work.

we use nagios (http://www.nagios.org/) to monitors our unix servers, and sms alerts are sent via an sms gateway.
